Job Summary Provides direct clinical social work services to residents and families, works collaboratively within an interdisciplinary care team, and facilitates support groups and therapeutic sessions. Also assists in program development, presentation of educational in services, and field instruction of Social Work students. Responsibilities Deliver direct clinical social work services to a designated case load in a case management format. Assist in orientation and training of staff under the direction of the Director of Social Work. Maintain required documentation in medical charts, databases, and reports in a timely manner to comply with accreditation standards. Coordinate benefit entitlements for residents through collaboration with state and veteran affairs offices. Provide occasional social work coverage for other units as needed. Participate as an integral member of the interdisciplinary patient care team. Facilitate support groups and therapeutic sessions with residents and families. Assist in program development and presentation of Residents' Bill of Rights, Family Visitor Council/Newcomer's group, and other facility in services. Support resident participation in bi monthly Resident Council meetings. Assist in field instruction of Social Work students as directed. Perform other duties as assigned. Required Qualifications Master's degree in Social Work from a program accredited by the Council on Social Work Education (CSWE). Two years of prior social work experience, which may include clinical graduate internship experiences. Licensed (LCSW) or license eligible MSW in the state of Georgia, with the license obtained prior to the end of a six month probationary period. Preferred Qualifications Experience in health care and long term care settings, gerontology, and end of life care. Experience with group work and program development. Knowledge, Skills & Abilities Proficient in Microsoft Office and relevant database systems. Knowledge of AU/GWVNH policies and procedures. Advanced graduate level social work foundation and skills working with individuals, groups, and families. Excellent interpersonal, verbal, and written communication skills. Strong attention to detail, organization, time management, and prioritization. Ability to work independently with minimal supervision and collaboratively as part of a team. Ability to maintain confidentiality and accurately report changes in patient conditions. Manual dexterity for accurate data entry into electronic medical records. Shift & Salary Shift: Days/M-F (may require work outside normal business hours) Pay Band: B12 - salary commensurate with qualifications within the established range.
