Job Summary

Under general supervision, the professional nurse adheres to the American Nurses Association (ANA) standards of professional nursing and performance, and the ANA Code of Ethics to provide care to all healthcare consumers. Applies critical thinking, evidence-based practices and competence to collaborate with the health care team to promote excellence in healthcare consumer safety and quality outcomes. Utilizes the Theory of Human Caring and Caritas in daily practice. "Jean Watson, PhD, RN, AHN-BC, FAAN, Founder / Director, Watson Caring Science Institute."

Facilitates the process of acceptance and transfer of external patients from multiple sources to ensure appropriate course of action.

Triage all Managed Care notification of patients in out of network hospitals.

Relays findings to multidisciplinary teams such as ADU and Hospitalists Admission Team.

Facilitates effective and appropriate use of healthcare resources for patients with complex medical and/or psychosocial needs. Understands members' eligibility and benefit coverage.

Review Clinical Notes and Case Presentation.

Facilitate and manage all outreach trauma patients into Denver Health including follow up

Information for Trauma Services.

Coordinate with Administrative Clinical Coordinator for a timely transfer and appropriate bed assignment.

Collaborate with all Critical Care, ALS, BLS transport agencies to coordinate transition of patients and resources within the continuum of care.

Acts a resource for any related departments regarding problem solving, conflict resolution and

Policy interpretation to ensure smooth operation of all services.

Assists nursing and physician staff in assessing patient condition, needs and family

Considerations for patient bed assignments.

Serves as a resource person for other staff to access when questions arise regarding intake

Information, scheduling conflicts, information/troubleshooting and difficult patient access issues.

Participates in the call review process of peers, periodic QA call review for complex transfers, and periodic evaluation of peers.

Provide insurance verification.

Coordinate with providers patient clinic appointments at specialty clinics for appropriate follow up care.

Provide emergent medical records after hours for HIM department.

    Responsibilities

    Maintain accurate, detailed reports and records Monitor, record and report symptoms and changes in patient's condition and vital signs Develop and modify patient care plans as indicated by patient's responses and conditions Modify patient treatment plans as indicated by patient's responses and conditions Consult and coordinate with health care team members to assess, plan, implement and evaluate patient care Order, interpret, and evaluate diagnostic tests to identify and assess patient's condition Monitor all aspects of patient care, including diet and physical activity Direct and supervise less skilled nursing/health care personnel Prepare patients for, and assist with examinations and treatments Assess the needs of individuals, families and/or communities to identify potential health or safety problems Instruct individuals and families on health education and disease prevention and after care Administer nursing care to ill, injured, convalescent or disabled patients Medication administration, documentation and reassessment Promote positive patient experience Other job duties as assigned per unit addendum

      Knowledge, Skills & Abilities

      Adherence to Standards of Professional Nursing Practice/ Process / Performance [ANA Standards addendum- signed] and accrediting body standards. Ensures all duties, responsibilities and competencies are conducted in a manner that is effective and appropriate to patients/clients to who care/service is being provided. Demonstrates knowledge and applicability of the principles of growth and development over the life span, as well as demonstrating the ability to assess data reflecting the patient's status and interpreting appropriate cultural information of the patient(s) to whom care/ services is being delivered/provided. Employee has completed and met their clinical competency standards Ability to use medical equipment and instruments to administer patient care. Skilled in maintaining effective working relationship with patients, employees, and the public. Ability to react calmly and effectively in emergency situations Ability to interpret adapt and apply guidelines and procedures Ability to communicate puters and Technology Microsoft Office Suite required. EPIC documentation preferred.

      High organizational and interpersonal communication skills. Possess excellent customer service standards and a customer-friendly voice.

      Able to critically think and multitask effectively, sometimes in a high stress situation.

      Understands admission procedures, EMTALA and insurance vetting.

      Be proficient with typing and working with electronic medical record systems and operate multiple phone line system.

      Be proficient with insurance standards.

      Ability to sort through all patient information and formulate an accurate picture of the patient and his/her condition for the MD's and Nurses.

      Ability to use computers and computer systems.


        Bilingual skills preferred.

        Demonstrates expertise in use of Epic System.

        Demonstrated and understanding of assessment findings as they relate to disease processes and continuum of care.

        Ability to follow all protocols and standard procedures for digital recorded calls.

          Education

          Associate's Degree (Required) Completion of an accredited nursing education program that satisfies the licensing requirements of the Colorado State Board of Nursing for Registered Nurses. Bachelor's degree in Nursing (BSN) preferred One year experience in a direct patient care setting preferred.

          Registered Nurse (Required) American Heart Association (AHA) Basic Life Support for Health Care Provider Certification/CPR required. Active Registered Nurse (RN) License in the State of Colorado or a participating compact state required.
