Job Summary and Qualifications
The Patient Navigator/Care Coordinator acts as the liaison for patients navigating the burn and reconstructive center's care journey. Responsibilities include:
- Coordinating care and transitional plans.
- Managing transportation and worker's compensation coordination.
- Ordering wound vacs and coordinating referrals.
- Ensuring smooth transitions back to school or work for patients.
- Connecting patients and families with multidisciplinary healthcare providers.
- Assessing patients’ medical, social, and psychosocial needs to facilitate access to resources.
- Providing education and support to empower patients in managing their healthcare.
- Conducting follow-up communications with patients and care teams.
- Tracking performance against HCA Service Line goals and identifying improvement opportunities.
Qualifications:
- Current Colorado Registered Nurse licensure.
- Basic Cardiac Life Support certification from AHA or ARC.
- Registered Nurse Diploma or Bachelor's Degree.
- At least 2 years of multidisciplinary clinical experience.
- Strong commitment to patient care and support.
- Proven proficiency in Microsoft Office and EMR systems.
- Excellent interprofessional communication skills.
- Able to multitask and maintain focus on goals.
- Clear understanding of patient navigation processes.
