Position Details: This is a part-time role, scheduled for Mondays, Wednesdays, and Fridays, focusing on the Haw Creek and Kennilworth areas.

As a Registered Nurse Home Health, you will collaborate with care management teams and health providers to develop and implement patient care plans, monitor outcomes, and ensure the best practices for patient health management.
Key Qualifications:
- Education: Registered Nurse with a Bachelor of Science in Nursing preferred.
- Licensure: Current RN license with the North Carolina Board of Nursing or eligible compact state license.
- Certification: Current BCLS certification required within the first week of employment.
- Experience: Minimum five years of clinical nursing experience; eight years preferred. Experience in case management or leadership roles is crucial.
- Bilingual candidates are preferred.
- Must obtain a driver’s license and maintain certification requirements within specified time frames.
