As a Registered Nurse Case Manager, you will work closely with interdisciplinary teams to ensure personalized care for our patients. Your responsibilities will include:
- Monitoring and following the established plan of care and adjusting treatments as necessary.
- Delivering direct patient care in accordance with the State Nurse Practice Act and hospice standards.
- Collaborating with the healthcare team to coordinate hospice services and address patient needs.
- Providing effective pain and symptom management.
Qualifications:
- Basic Cardiac Life Support certification within 30 days of employment.
- Current Class C License.
- Registered Nurse license.
- Associate Degree in nursing.
